Additionally, the distribution of application forms for the scheme has commenced in villages. Eligible families in the state with an annual income of up to ₹8 lakh, as well as government employees and pensioners, will be covered under this scheme.

However, senior citizens aged 70 or older have been excluded from this scheme, as they are already eligible for benefits under the ‘Ayushman Bharat Yojana’. Medical services under this scheme will be provided on a completely cashless basis; consequently, upon completion of the registration and verification processes, every eligible family will receive a health card in accordance with the regulations.

  • Ration Card will be required to verify the number of family members; this will facilitate the process of issuing a Health Card for the family.
  • Photographs showing the head of the family along with the other family members must be provided.
  • An income certificate stating that the family’s annual income is ₹8 lakh or less must be submitted.
  • Documents such as a Voter Card, electricity bill, etc., may be provided as proof of residence or address in West Bengal.
  • The applicant must provide their own signature.
  • An active mobile number must be provided for verification purposes and to receive various updates regarding the scheme; subsequent updates concerning the card may also be received via SMS on this number.

How to Apply Mukhyamantri Swasthya Bima Yojana West Bengal

  • The online application process for the Chief Minister’s Health Insurance Scheme has not started yet. So, currently there is no need to go to the portal and fill the form to apply online. The detailed application process will be informed on the official website of MMSBY when the online application starts.
  • However, the distribution of application forms for this scheme has already started in rural areas. The health center staff or related staff are delivering the application forms door to door.
  • These application forms are not ordinary blank forms. Application forms with names are being printed for families who have ration cards, especially for families holding RKSY-I and RKSY-II ration cards.
  • Collect the application form with names from the concerned staff.
  • Verify your name and other family information given in the form carefully.
  • Fill/attach the required information and documents as per the instructions in the form.
  • Verify the information of the family members correctly.
  • Submit the form to the concerned health center or responsible staff.
